PUTTI-PLATT
REPAIR FOR RECURRENT ANTERIOR
DISLOCATION OF SHOULDER
Make
skin incision along medial border of
deltoid from
pectoralis major to
coracoid process. Then continue the line
of incision laterally to reach the
acromion
Expose
the deltopectoral groove and ligate the
cephalic vein. Find the
thoracoacromial
arterial branches and divide them
Secure
the
deltoid and divide it 1cm from its
insertion to the clavicle. Then turn the
deltoid,skin and subcutaneous tissue
laterally to expose deeper stratum. Then
secure the
coracobrachialis and short
head of
biceps which are divided on
their lateral side to expose the
subscapularis above and
teres major
below
Place
stay stitches on
subscapularis
and divide it to expose the capsule of
shoulder joint.
Overlap
the anterior capsule, which is lax and
reinforce it with subscapularis. This is
the Putti-Platt procedure.
