Cardiopulmonary resuscitation is applied to a patient who has just suddenly developed cardiac arrest. That means the patient has lost his/her pulse and subsequently his /her respiration. Modern cardiopulmonary resuscitation or CPR involves mainly delivery of punches on the precordium in order to force the arrested heart to pump its blood at a rate of 100 beats per minute for more than 5 minutes. Only after this can artificial respiration begin with a puff in 5 minutes. This is because it is now well recognized that the brain can survive lack of oxygen for longer than 5 minutes if heart is pumping its blood through the systemic circulation since oxygen does not fall to zero immediately after cardiac arrest. This best example to illustrate this is the failure to initialize respiration in the new born which may extend well beyond 30 minutes in some instances. During artificial respiration, the nose is closed off with the index finger and thumb and the chest must rise as the exhaled air is delivered.
