History taking

History taking is the art of asking questions from a patient so that he/she can say exactly how he /she feels. Only then can the doctor understand the problem(s) of the patient. The above schedule is followed in modern history taking from obtaining biodata to presenting complaint to past medical history to treatment, family, social and psychiatric histories. What the patient complains of is described as symptom.

  1. Say hello and introduce yourself
  2. Ask for name and complaint
  3. List the complaints in order of severity and duration
  4. Ask the patient to tell the story of his/her illness in his own words
  5. Ask questions relating to each system
  6. Ask in relation to past, treatment, family, social and psychiatric histories.
